Subcontractor Tax Information

As part of the onboarding process, approved subcontractors are required to complete an IRS Form W-9. 

1.  Print the W-9 form, complete it, and bring the completed form with you to your scheduled onboarding appointment; or

2.  Complete a W-9 provided by Sida's Janitorial Operations in person during your onboarding appointment.
